ROLE SUMMARY
To be an active member of this domiciliary rehabilitation team, providing short term, high
quality rehabilitation in a community environment. This will mean working without direct
supervision carrying out tasks identified via personalised plans of care under the direction of
the Nurses, Therapists and Associate Practitioners within the team.
The team strive to promote independence and recovery following discharge from hospital, or
to prevent unnecessary hospital admission by providing the right patient with the care as the
right time.
JOB SUMMARY
Delivering a compassionate, dignified and respectful service to patients at all times
Ensuring the values outlined in the NHS constitution are adhered to daily and any matter of
concern are raised with the relevant line manager or through the necessary processes within
the Trust.
SERVICE PROVISION
 Undertake through individual care plans, personal, practical and rehabilitative duties
as set by staff including nurses, therapists and associate practitioners.
 Work as a member of this multidisciplinary team observing, recording and reporting
any changes in a patient’s condition or circumstances
Open Compassionate Empowering
 Be aware of home safety – reporting an hazards to the patient or staff to the shift
leader so appropriate action can be taken
 Be familiar with the steps necessary in an emergency and be able to act accordingly
 Ensure the needs of carers and relatives are listened too and incorporated into
service needs by reporting these back to shift leaders
 Apply knowledge/skills gained from education/training so that high care standards
are maintained at all times
SERVICE DEVELOPMENT
 Participate in ongoing personal development and training including all aspects of
mandatory training as scheduled
 Manage resources including equipment and supplies in an economical way to
prevent waste, while ensuring standards pertaining to infection control and safety are
met at all times
 Be aware of the standards related to the Care Quality Commission and how this post
can support the evidence of compliance to these standards
 Develop and maintain links with other community staff to promote integrated working
 Be aware of current NHS initiatives and how these may affect the team and service
provision
 Ensure own practice is of a high standard at all times
INFORMATION, COMMUNICATION AND RECORDING
 Record accurately in line with record keeping policy in both written and electronic
records as necessary
 Ensure excellent communication is achieved and maintained with patients,
colleagues and the pubic at all times
 Attend team meetings as often as possible to ensure understanding, and to assist in
the promotion of a team where members are respected and involved at all times
 Ensure all individuals whether patients, colleagues or members of the public are
treated with respect and dignity at all times
 Respect the personal, religious and cultural beliefs of patients and their carers at all
times

 Respect patients property and environment at all times
 Maintain confidentiality at all times in accordance with policies and procedures
 Ensure all duties are undertaken in line with health and safety policies and
procedures
 Ensure accurate records are maintained regarding hours worked, mileage and
annual leave in line with Trust policy and local team arrangements
Participate in the Trusts clinical supervision and appraisal programme
ADDITIONAL DUTIES
In addition to the above duties you will also be expected to perform the below key activities
in line with your job role;
- Complete mandatory training in line with Trust policy and procedures
- To participate in the staff appraisal process and to undertake for any staff you
manage
- To keep yourself updated on all matters relating to Trust policy
- To provide management supervision where appropriate
- You will be expected to work collaboratively with key partner organisations, service
users, carers, clinicians and other practitioners within the multi-disciplinary team in
delivering services; providing a 7-day a week, 24-hours a day service for 365 days a
year working shifts, where appropriate and operationally required.
